First Steps

Call us or drop by to schedule your first appointment.

You will need to come in approximately forty minutes early to your first appointment to fill out New Client paperwork.

You will need to bring the following items to your first appointment:
?Insurance/benefits card (private, Medicare, Medicaid or Equality Care)
?Social Security number
?Government provided photo ID (driver?s license, State-issued ID card, military ID)
?Proof of income (pay stubs, tax returns, social security disability statement)
?Court documents (if applicable)
